Researchers at Charité – Universitätsmedizin Berlin have found that Dronabinol, a medical cannabis compound containing THC, provides relief for patients suffering from post-traumatic stress disorder (PTSD) nightmares. Published in the journal Nature Medicine, the double-blind, placebo-controlled study offers a potential solution for a debilitating symptom that existing psychiatric medications often fail to treat.
PTSD can develop after exposure to severe traumatic events, such as natural disasters, serious accidents, violence, or wartime experiences. Affected individuals often relive these events intensely at night, leading to extreme sleep deprivation and a fear of going to bed. While antidepressants or blood pressure-lowering medications are commonly prescribed, they are only partially effective and rarely stop the nightmares. Currently, Germany has no approved medication specifically indicated for trauma-related nightmares.
How THC Targets Sleep and Emotional Memories
Prof. Stefan Röpke and his team at Charité’s Department of Psychiatry and Neurosciences focused their research on tetrahydrocannabinol (THC), the primary psychoactive compound in cannabis. THC interacts with the body's endocannabinoid system, which regulates sleep, stress, and emotional memory processing.
"There is evidence suggesting that during REM sleep—the intense dream phases in which the brain processes experiences and regulates emotions—THC reduces dream activity and thereby alleviates nocturnal stress responses," Prof. Röpke explained.
In Germany, cannabis-based medications have been legally permitted for medical use since 2017. For this study, the researchers selected Dronabinol, a prescription drug derived directly from the cannabis plant and administered in the form of oral drops.
The Clinical Trial and Patient Outcomes
The study evaluated more than 170 patients diagnosed with PTSD who experienced frequent, intense nightmares. Over a ten-week period, participants received either Dronabinol or an identical-looking, cannabis-flavored placebo every evening before bedtime. Neither the healthcare providers nor the participants knew who received the active drug.
By the end of the trial, patients in the Dronabinol group experienced a substantial reduction in nightmare frequency and intensity compared to the placebo group. The data showed clear differences in patient recovery across several metrics.

While the compound specifically targeted nightmares and improved sleep quality, it did not alter other PTSD symptoms, such as daytime anxiety or accompanying depression. However, by interrupting the tormenting nightly cycle of reliving trauma, the medication allowed patients to get restful sleep, which substantially improved their overall well-being.
Safety, Side Effects, and Next Steps
The research team monitored participants for adverse effects and dependency. Dronabinol did not cause any severe side effects. Some participants reported mild to moderate reactions, including dizziness, headaches, and increased appetite.
Importantly, the researchers observed no withdrawal symptoms after the ten-week treatment period ended and patients stopped taking their nightly doses. The next phase of research will focus on whether THC remains safe and effective over longer periods, or if patients develop a tolerance to the compound over time.
