    Cannabis, Cocaine, and Amphetamines Linked to Higher Stroke Risk

    A Cambridge meta-analysis reveals how cocaine, amphetamines, and cannabis elevate stroke risk, especially in young adults.

    Researchers at the University of Cambridge have linked the use of cannabis, cocaine, and amphetamines to a heightened risk of stroke. Analyzing data from over 100 million people, the study identifies these common recreational drugs as preventable risk factors for both ischemic and hemorrhagic strokes, particularly among younger adults.

    A stroke occurs when the blood supply to part of the brain is interrupted by a clot (ischemic) or when a blood vessel bursts (hemorrhagic). Deprived of oxygen, brain cells begin to die within minutes, making rapid medical intervention critical. While age and family history are unchangeable risk factors, lifestyle choices like smoking and diet are preventable. The Cambridge team sought to determine if recreational drug use should be categorized alongside these preventable risks.

    Quantifying the Risk: Cocaine, Amphetamines, and Cannabis

    The study, published in the International Journal of Stroke, pooled results from multiple historical investigations. By combining these datasets, the researchers established clear statistical associations between specific substances and stroke occurrence.

    Cocaine and amphetamines showed the strongest correlations. Cocaine use was associated with a 96% increase in stroke risk, meaning users are nearly twice as likely to experience a stroke compared to non-users. Amphetamine use carried a 122% increase in risk. Cannabis showed a lower but still notable association, elevating stroke risk by approximately 37%.

    In contrast, the researchers did not find a statistically clear increase in stroke risk associated with opioid use. However, the authors noted that opioids carry other severe health dangers, including addiction and fatal overdose, meaning they cannot be considered safe.

    Severe Implications for Adults Under 55

    While stroke is more common in older populations, it can occur at any age. When younger adults experience a stroke, the long-term consequences are often severe, potentially leading to decades of disability. The Cambridge study highlighted worrying trends for this demographic.

    Among individuals younger than 55, amphetamine use was linked to a 174% increase in stroke risk. This means younger users face nearly triple the risk of non-users. Cocaine use in the under-55 group was associated with a 97% higher risk, while cannabis was linked to a 14% increase.

    Biological Mechanisms and Genetic Evidence

    To determine if the drugs directly cause strokes rather than just appearing alongside other high-risk behaviors, the researchers analyzed naturally occurring genetic differences associated with drug use. This genetic analysis supported a causal role for both cocaine and cannabis use disorders.

    Specifically, cocaine use disorder was linked to brain bleeding (hemorrhagic stroke) and strokes caused by blood clots originating in the heart. Cannabis use disorder was linked to overall stroke risk, showing a connection with strokes affecting large arteries.

    Several biological mechanisms explain how these substances damage the brain's vascular system:

    • Blood Pressure Spikes: Cocaine and amphetamines cause sudden, sharp increases in blood pressure, putting immediate stress on blood vessel walls.
    • Vessel Spasms: Certain drugs cause blood vessels to constrict or spasm, severely restricting oxygen delivery to brain tissue.
    • Heart Rhythm Disruptions: Drug-induced irregular heartbeats can cause blood clots to form in the heart, which can then travel to the brain and block arteries.
    • Vascular Inflammation: Amphetamines are linked to the inflammation of blood vessels, increasing the likelihood of long-term vessel damage and rupture.

    Study Limitations and Future Outlook

    The researchers cautioned that the findings require careful interpretation. Because a meta-analysis relies on existing studies, it cannot fully account for variations in drug purity, dosage, frequency of use, or the simultaneous use of multiple substances.

    Additionally, the genetic analysis focused primarily on drug use disorders, which reflect heavy or chronic use rather than occasional recreational exposure. A similar genetic analysis could not be performed for amphetamines due to a lack of large genetic datasets.

    Despite these limitations, the scale of the study provides strong evidence that recreational drug use is a key preventable risk factor for stroke. The findings suggest that avoiding these substances is an important step in reducing the global burden of stroke, particularly among younger populations.
